You and Your Provider

take this test. Depending on the test, this may be your physician, neuropsychologist,
certified athletic trainer, school psychologist, or other qualified professional.
Your provider may administer the test personally or assign another person
to supervise the test administration. Your provider is responsible for
interpreting and discussing your test results with you.
Personal Identifying Information
HeadMinder does not share personal identifying information with
third parties. Personal identifying information is only collected as it is
necessary to help your test provider evaluate your test results. Your information is stored so it
can be compared to future tests that you may take at the request of your provider. If you have
additional questions about the type of personal identifying information that will be collected

Aggregate Data
Aggregate data is combined data from all test takers and does
not include your name or any other information that could identify you. We may analyze
and/or share this data with third parties. Analysis of aggregate data may expand the understanding
of illnesses and injuries that affect the brain, which may lead to new treatments.
Who Has Access to Test Results
Your test provider (the person responsible for your care who requested you take
this test) has access to your test results only with a secure password. Further access to your test
results is determined by the records management policies of your provider's organization, which is
subject to professional standards and applicable federal and state laws regarding the confidentiality
of health, educational, and other records. At the request of your provider or your provider's
organization, appropriate employees of HeadMinder may access your records only as needed to assist
your provider with technical difficulties or questions regarding test validity.
HeadMinder does not release test results or personal identifying information to
third parties without your written permission.
HeadMinder does not provide test takers with direct access to test results because
all tests must be interpreted by a provider with appropriate education, training, and experience in
the relevant area of assessment. If you would like a copy of your test results, please discuss it
with your provider.
As Required by Law
The release of health, educational, and other records is subject to applicable
federal and state laws and regulations. To the extent required by law or the judicial process,
your provider or HeadMinder may be required to release your records to authorities to serve a clear
public interest or to fulfill a public duty.
